Insurance

Zehr Center for Orthopaedics participates with the following health insurance plans. Covered procedures vary with each insurance company.

Patients are responsible for deductibles, co-payments and remaining balances and are expected to keep paid up at each encounter.

If you do not see your insurance listed below, or have any questions, please contact our office and our staff will assist you in.

Payment at Time of Service

If you do not have medical insurance or are involved in a liability case, payment in full is expected at the time of service. We do not bill attorneys, nor do we wait for final outcome of your case. Although not encouraged, at times it may be necessary to rely upon a “payment plan” which can be negotiated with our business manager upon request.

You will be requested to sign our Financial Responsibility Policy prior to your first visit with us so that your financial obligations to the Zehr Center for services rendered is quite clear. Download our Financial Policy.
